چکیده

Purpose of review: It was recently reported that a lung protective ventilation strategy resulted in no fatalities in 17 patients with ARDS secondary to lung contusion. Although the efficacy of a lung protective ventilation strategy to reduce mortality in ARDS patients has been demonstrated, the underlying mechanisms for this reduction remain unclear. Recent findings: ARDS is characterized by an inflammatory state of the lung that can be aggravated by mechanical ventilation. Especially mechanical ventilation allowing cyclic opening and closing of unstable alveoli, enhances cytokine release and can even result in loss of compartmentalization. Disbalance in the cytokine response can promote bacterial colonization Case Report

Modeling the dynamics of recruitment and derecruitment in mice with acute lung injury.

Lung recruitment and derecruitment contribute significantly to variations in the elastance of the respiratory system during mechanical ventilation. However, the decreases in elastance that occur with deep inflation are transient, especially in acute lung injury. Bates and Irvin (8) proposed a model of the lung that recreates time-varying changes in elastance as a result of progressive recruitme...

Effects of recruitment/derecruitment dynamics on the efficacy of variable ventilation.

Variable (or noisy) ventilation (VV) has been demonstrated in animal models of acute lung injury to be superior to constant (or conventional) ventilation (CV), in terms of improved gas exchange and mitigation of lung injury, for reasons that are not entirely clear. We hypothesized that the efficacy of VV is related to the fact that recruitment and derecruitment of lung units are dynamic process...

Investigation of histopathological and radiological effects of surfactant treatment in an experimental female rat model of lung contusion

Objective(s): Pulmonary contusion (PC) is a clinical entity that often accompanies blunt traumas. We aimed to investigate the radiological and histopathological effects of surfactant treatment in an experimental rat model in which lung contusion was formed by blunt thoracic trauma.Materials and Methods: 50 female Sprague-Dawley rats were...

Preliminary study of ventilation with 4 ml/kg tidal volume in acute respiratory distress syndrome: feasibility and effects on cyclic recruitment - derecruitment and hyperinflation

INTRODUCTION Cyclic recruitment-derecruitment and overdistension contribute to ventilator-induced lung injury. Tidal volume (Vt) may influence both, cyclic recruitment-derecruitment and overdistension. The goal of this study was to determine if decreasing Vt from 6 to 4 ml/kg reduces cyclic recruitment-derecruitment and hyperinflation, and if it is possible to avoid severe hypercapnia. METHOD...

Derecruitment Test and Surfactant Therapy in Patients with Acute Lung Injury

Introduction. A recruitment maneuver (RM) may improve gas exchange in acute lung injury (ALI). The aim of our study was to assess the predictive value of a derecruitment test in relation to RM and to evaluate the efficacy of RM combined with surfactant instillation in patients with ALI. Materials and Methods. Thirteen adult mechanically ventilated patients with ALI were enrolled into a prospect...
